Compile a List of Facilities

Arrange visits to the selected facilities, allowing the individual
and their family to tour the communities, meet staff, and evaluate the amenities and

Application and Admission
Once the individual has selected a preferred facility, the application and admission process
begins.
Submit Application
Complete and submit the facility’s application paperwork, including
medical records and any required assessments.
Review and Sign Contract
Carefully review and sign the contracts, including the lease
agreement and financial arrangements.
Coordinate Move-In Logistics
Plan the logistics of the move, including the move-in date,
transportation, and any necessary arrangements for the individual’s belongings.
Transition and Ongoing Support
After admission, the focus shifts to ensuring a smooth transition and ongoing support.
Orientation and Welcome
