Our Aproach

Initial Assessment and Information Gathering

In this stage, we’ll gather essential information about your individual’s needs, preferences and circumstances. This initial assessment helps determine which Senior Living facility is the right choice

Conduct a Needs Assessment

Evaluate the individual’s physical, cognitive, and emotional needs. Assess their mobility, daily living skills, medical conditions, and any special requirements.

Determine Budget

Discuss the individual’s financial situation, including their budget for
Senior living. This will help narrow down options within their price range.

Identify Location Preferences

Ask about location preferences, considering proximity to family, medical facilities, and personal preferences for urban or rural settings.

Assess Lifestyle and Cultural Preferences

Inquire about dietary restrictions, religious practices, language preferences, and any other lifestyle or cultural considerations.

Research and Selection

Once we have a clear understanding of the individual’s needs and preferences, our team will research and select a suitable senior living facility.

Compile a List of Facilities

Research and create a list of Senior living facilities that align with the individual’s requirements and preferences.

Schedule Tours and Visits

Arrange visits to the selected facilities, allowing the individual and their family to tour the communities, meet staff, and evaluate the amenities and services offered.

Application and Admission

Once the individual has selected a preferred facility, the application and admission process begins.

Submit Application

Complete and submit the facility’s application paperwork, including medical records and any required assessments.

Review and Sign Contract

Carefully review and sign the contracts, including the lease
agreement and financial arrangements.

Coordinate Move-In Logistics

Plan the logistics of the move, including the move-in date,
transportation, and any necessary arrangements for the individual’s belongings.

Transition and Ongoing Support

After admission, the focus shifts to ensuring a smooth transition and ongoing support.

Orientation and Welcome

Upon arrival, the individual should receive a warm welcome and orientation to the facility, including introductions to staff, residents, and the daily routine.

Personalize Living Space

Help the individual personalize their living space with familiar belongings to create a sense of comfort and familiarity

Coordinate Healthcare

Ensure that medical care and medication management are ongoing as required, and facilitate communication between the facility’s healthcare team and the individual’s primary healthcare providers.

Engage in Activities and Socialization

Encourage participation in facility activities and social events to foster a sense of community and well-being.
